This video demonstrates a case of treatment of recurrent pilonidal sinus disease after 3 unsuccessful surgery in a young patient with clinically significant hypercortisolism. The technical features of the Limberg flap procedure are explained in details.
Limberg flap is versatile rhomboid Limberg flap. This flap was designed by AA Limberg of Leningrad.This flap can close even the large size defect of the face. The flap has almost same thickness and color as of the defect area with very good vascularity. Basically it is a parallelogram with two angles of 120 degree and two angles of 60 degree. However these angles can be modified depending on the shape and size of the lesion. This patient had histopathological proved basal cell carcinoma. The flap design of parallelogram was marked as shown in video with the marker pen. The 2% xylocaine with adrenaline was infiltrated at the proposed incision lines. With clear safety skin margin of 0.5 cm around the lesion, the basal cell carcinoma was excised in a rhomboid shape. The incision line was extended to create flap as per designed marking. The defatting of tissue was done with sharp curved scissors. The flap was rotated to cover the earlier defect and sutured with nonabsorbable suture material.
𝐏𝐈𝐋𝐎𝐍𝐈𝐃𝐀𝐋 𝐒𝐈𝐍𝐔𝐒 : 𝐋𝐈𝐌𝐁𝐄𝐑𝐆 𝐅𝐋𝐀𝐏 Anesthesia : Spinal anesthesia Position : Prone with buttocks strapped apart Operative steps; A rhombic area of skin is marked over pilonidal sinus involving all midline pits and lateral extensions if any. The long axis of the rhomboid in midline is marked as A-C, c being adjacent to perianal skin, A placed so that all diseased tissue can be included in the excision. The line B-D transects the mid point of A-C at right angles and is 60% of it’s length. D-E is a direct continuation of the line B-D and is of equal length to the incision B-A, to which it will be sutured after rotation. E-F is parallel to B-C and of equal length. After rotation it will be sutured to A-D. The skin & subcutaneous flap to be removed is excised down to deep fascia, and a rhomboid area of specimen including Pilonidal sinus and it’s all extension are removed. The flap is raised so that it includes skin , subcut fat, and the fascia overlying gluteus maximus, rotated to cover midline rhomboid defect. The defect thus created can be closed in linear fashion. Das Areal hat eine bewegte Vergangenheit hinter sich. Während des Zweiten Weltkrieges befanden sich hier auf dem südlichen Teil des die Teutowerke auf dem Munition für den Krieg produzierte. Ab 1945 nutzten die Britischen Streitkräfte das Gebiet und bauten es für ihre Zwecke um. Die heutige zivile Nutzung umfasst u.a. Musiker der verschiedensten Richtungen, Sportclubs und viele weitere Nutzer.
